class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Kinesiolog-vejledt funktionel tr\u00e6ning som supplement til intradialytisk cyklingsprogram hos patienter med nyresygdom i slutstadiet: et randomiseret kontrolleret fors\u00f8g<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-357357b elementor-widget-divider--view-line elementor-widget elementor-widget-divider\" data-id=\"357357b\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"divider.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<div class=\"elementor-divider\">\n\t\t\t<span class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p><\/p><p class=\"has-text-color\">Dette var et prospektivt, randomiseret, kontrolleret interventionsfors\u00f8g, der sammenlignede to strategier for tr\u00e6ningsordinering og r\u00e5dgivning hos pr\u00e6valente HD-patienter. Treoghalvfjerds HD-patienter blev kontaktet i HD-enhederne p\u00e5 Universitetsmedicinsk Center i Ljubljana, Slovenien. Inklusionskriterierne var f\u00f8lgende: nyresygdom i slutstadiet, nyreerstattende behandling med HD&gt;3 m\u00e5neder, alder 18-90 \u00e5r, i stand til selvst\u00e6ndig gang og spisning, i stabil medicinsk tilstand. Kriterier for ikke-inklusion var: kronisk ondartet eller infekti\u00f8s sygdom, ukontrolleret arteriel hypertension med et gennemsnit af de sidste fem pr\u00e6-dialyse blodtryksv\u00e6rdier over 180\/100 mm Hg, angina pectoris af Canadian Cardiovascular Society grad 2-4, New York Heart Association hjertesvigt grad 3 eller 4, tilstedev\u00e6relse af psykotisk sygdom eller mental handicap, tidligere amputationer (mere end 2 fingre p\u00e5 underekstremitet og\/eller mere end 2 fingre p\u00e5 overekstremitet) eller enhver anden tilstand, der kunne for\u00e5rsage klinisk ustabilitet hos patienten (f.eks. gentagne gastrointestinale bl\u00f8dninger, levercirrose med hyppige forv\u00e6rringer, fremskreden demens med d\u00e5rligt samarbejde fra patienten).   <\/p><p> <\/p><p class=\"has-text-color\">Studiets eksklusionskriterier (tilbagetr\u00e6kning) omfattede enhver interkurrent sygdom eller traume, der forhindrede patienten i at forts\u00e6tte med tr\u00e6ningsprogrammet i en periode l\u00e6ngere end 14 dage, forekomst af akut sygdom varende mere end 3 uger eller sluttende mindre end 3 uger f\u00f8r studiets afslutning, eller hvis patienten havde f\u00e5et ordineret medicin for det ved studiets afslutning, diagnose af ondartet sygdom under forskningsforl\u00f8bet og tilbagetr\u00e6kning af samtykke til deltagelse. National Medicinsk Etisk Komit\u00e9s godkendelse (Sundhedsministeriet, Republikken Slovenien, godkendelsesdokument nummer 0120-97\/2017-3 KME 68\/03\/17) og skriftligt informeret samtykke blev indhentet i alle tilf\u00e6lde. Studiet overholder Helsinki-deklarationen, som revideret i 2013. Studiet blev registreret p\u00e5 ClinicalTrials.Gov (Clinicaltrials.gov identifikator: NCT03334123) den 7. november 2017.    <\/p><p> <\/p><p class=\"has-text-color\">Det prim\u00e6re endepunkt var 10 gentagelser rejse-s\u00e6tte-sig test tid (STS-10) ved uge 8, som m\u00e5ler underkroppens styrke<sup>19<\/sup>. Begrundelsen for valget af dette endepunkt var baseret p\u00e5 vores tidligere forskning, der viste denne tests overlegenhed i f\u00f8lsomhed over for ur\u00e6mieffekter, styrken af sammenh\u00e6ngen med dagligdags aktiviteter og et relativt stort underskud ved denne test hos dialysepatienter sammenlignet med andre fysiske pr\u00e6stationstests<sup>17,20<\/sup>. <\/p><p> <\/p><p class=\"has-text-color\">De sekund\u00e6re endepunkter var: den aerobe kapacitet vurderet ved seks-minutters gangtest<sup>21<\/sup>, h\u00e5ndgrebsstyrke<sup>22<\/sup> vurderet med kalibreret hydraulisk h\u00e5nd dynamometer (Jamar, Patterson Medical, Warrenville, Illinois), underkroppens fleksibilitet ved sid-og-r\u00e6k test<sup>23<\/sup>, balance ved Stork test<sup>20<\/sup> p\u00e5 en skumpude (Airex, Sins, Schweiz) og overkroppens fleksibilitet med rygkrads test<sup>24<\/sup>. Alle tests blev udf\u00f8rt som tidligere beskrevet<sup>17,25,26<\/sup> i en fast r\u00e6kkef\u00f8lge for at minimere patienttr\u00e6thed. F\u00f8r testene m\u00e5lte vi h\u00f8jde, v\u00e6gt og kropssammens\u00e6tning ved hj\u00e6lp af bioimpedansanalyse (Body Composition Monitor, Fresenius AG, Bad Homburg, Tyskland). Den f\u00f8rste funktionelle test var en seks-minutters gangtest efterfulgt af h\u00e5ndgrebsstyrketest, sid-og-r\u00e6k test, balance Stork test og rygkrads test. Det tog cirka 40 minutter for en patient at gennemf\u00f8re alle resultatvurderinger (inklusive hvileperioder). Testningen blev udf\u00f8rt p\u00e5 dialysefrie dage: patienter, der var i dialyse tirsdag-torsdag-l\u00f8rdag, blev testet om fredagen, og patienter, der blev dialyseret mandag-onsdag-fredag, blev testet om l\u00f8rdagen. Resultater blev vurderet p\u00e5 tre tidspunkter: f\u00f8r interventionen (T1), efter otte uger (T2, slutningen af fase 1) og efter 16 uger (T3, slutningen af fase 2). De samme bed\u00f8mmere blev tildelt individuelle endepunktvurderinger p\u00e5 alle tidspunkter og var blindede for behandlingstildeling. Patienter og dialysecentrets plejepersonale var ikke blindede.         <\/p><p> <\/p><p class=\"has-text-color\">Efter baseline-testning blev patienterne randomiseret ved hj\u00e6lp af et computerprogram og fordelt i forholdet 1:1 til den eksperimentelle gruppe (EXP) eller den aktive kontrolgruppe (CON). Vi skjulte fordelingen for patienter og dialysepersonale f\u00f8r studiets start ved at inkludere og fordele patienter i henhold til en liste, som kun blev vurderet af forskerne. I de f\u00f8rste otte uger af interventionen (fase 1) udf\u00f8rte den eksperimentelle gruppe vejledt funktionel tr\u00e6ning ledet af en kinesiolog f\u00f8r hver HD-procedure.  <\/p><p> <\/p><p class=\"has-text-color\">Under disse sessioner modtog de r\u00e5dgivning om at mestre de funktionelle tr\u00e6ningsrutiner pr\u00e6cist og overf\u00f8re disse f\u00e6rdigheder til hjemmemilj\u00f8et i studiets anden fase. Desuden udf\u00f8rte de et cyklingsprogram under dialyse p\u00e5 den tilpassede ergometer (Model B&#8217;ft Mini, Lemco, Danmark) i den f\u00f8rste halvdel af dialyseproceduren. <\/p><p> <\/p><p class=\"has-text-color\">Kontrolgruppen udf\u00f8rte kun det intradialytiske cyklingsprogram med samme instruktioner som den eksperimentelle gruppe. Specifikt blev intradialytisk cykling overv\u00e5get af den samme kinesiolog, som sigtede mod kontinuerligt at \u00f8ge cyklingsbelastningen (modstand) eller tiden med opretholdelse af den opfattede anstrengelsesgrad p\u00e5 4.-5. grad p\u00e5 en 10-graders Borg-skala. <\/p><p> <\/p><p class=\"has-text-color\">Initial intradialytisk cyklingsvarighed blev sat til 15 minutter med en gradvis stigning i tid og intensitet for at n\u00e5 en varighed p\u00e5 op til 60 minutter. Hastigheden af stigning i belastning og varighed blev individualiseret i henhold til hver patients vurderede opfattede anstrengelse (RPE) respons og deres motivation. Denne ordinationsstrategi var den samme i begge studiegrupper. RPE blev vurderet ved hj\u00e6lp af Borg 10-graders skalaen, som er nyttig for   <\/p><p> <\/p><h3 id=\"conclusions\" class=\"has-text-color\">Konklusioner.<\/h3><p> <\/p><p class=\"has-text-color\">I dette studie unders\u00f8gte vi effekten af funktionel tr\u00e6ning tilf\u00f8jet til grundl\u00e6ggende intradialytisk cykling og unders\u00f8gte gennemf\u00f8rligheden og b\u00e6redygtigheden af funktionel tr\u00e6ningsoverf\u00f8rsel til et ikke-overv\u00e5get hjemmemilj\u00f8. Resultaterne demonstrerer signifikant overlegenhed af den eksperimentelle tr\u00e6ningsstrategi vedr\u00f8rende det prim\u00e6re studiem\u00e5l forbundet med pr\u00e6station af underekstremiteterne. Overlegenhed i balance, h\u00e5ndgrebsstyrke og overkropsfleksibilitet blev ogs\u00e5 vist.  <\/p><p> <\/p><p class=\"has-text-color\">Selvom der blev fundet signifikante intra-gruppe forbedringer i 6-minutters gangtest distance i den eksperimentelle gruppe, n\u00e5ede effekterne ikke statistisk signifikans sammenlignet med kontrolgruppen.
